Cell Phone Policies and Procedures MDUSD

Below are links to the policies and procedures for cell phone usage by MDUSD employees.

  • Policy 3513.1 for Use of Cellular Phones (This document includes Cell Phone Allowance Form) (PDF)
  • Cell Phone Procedures List for Employees (PDF)
  • Annual Cell Phone Allowance Authorization Form (PDF)
  • CL to IL Subscriber Info Form B Revised 10/29/07 (EXCEL)
Employees may return the Nextel phone that was issued to them by the district or they may keep the Nextel phone which will allow them to only use the “Direct Connect or Push to Talk Feature”. You can use the corporate cell phone number that came with the phone as your personal cell number as an option if prefer not to purchase your own cell phone. You may still request approval for a stipend. For this process you must complete the attached Sprint/Nextel (the district cell carrier) credit application.

THIS FORM IS FOR EMPLOYEES WITH DISTRICT NEXTEL PHONES THAT WANT TO STAY WITH “SPRINT/NEXTEL”

The Sprint/Nextel credit application must be completed before they can port the district cell number to a personal cell number. Please forward the completed application by email it to this email address: loyaltycilsprint@sprint.com

Please note: that if you do not want to use Sprint/Nextel as your carrier, then you will not fill out this application and you will have to purchase your own cell phone and complete the required paperwork for approval in order to receive your monthly stipend. You may, however, still use your District Nextel phone for the “Direct Connect or Push-to-Talk Feature” ONLY.
