The General Pick-up Delivery Form [PDF format] is a general purpose form that can be used to notify the warehouse for the pick-up of items such as discard books, equipment and furniture description. The following instructions only apply to the use of this form with the exception of equipment and furniture. Use the General Pickup/Delivery Form (without surplus forms) for the following:
- Discard books to be picked up by warehouse
- Supplies from one site to another
- Free items available on this site
- Excess furniture or equipment available on this site
- Teacher moves
- Pianos for disposal or moves within a site
- Printer cartridges going to warehouse for recycle
