Standardized permission slips have been developed to be used by all sites (elementary :: middle :: high). Forms must be completed by the parent/guardian prior to any student participating in a field trip. The permissions slips should be kept with the instructor in charge at all times during the event. A new permission slip must be completed for the District Warehouse (elementary form-#00724332 :: middle school form-#00724334 :: high school form-#00724336). Copies of the elementary school permission slips are also available in Spanish by calling the Office of General Counsel.
