The classified Application Form was created in Adobe Acrobat PDF format.  To use all the features in this application, you will need to download and install Adobe Acrobat Reader 8.  The latest version of Acrobat Reader is available free from Adobe’s website.  If needed, the link for Acrobat Reader is on the application download page.  This version of Acrobat Reader 8 allows you to fill out the form, print the form, and save the filled out application on your computer.

A separate application form must be submitted for each position for which you are applying.  Resumes and other documentation can be attached but may not be substituted for the official Mt. Diablo Unified School District application form.  Some positions also may require a supplemental application.



All applications must be filed in the Personnel Services offices no later than 4:00 PM on the final filing date, as listed on the Job Announcement.  Late applications and incomplete application packets will be disqualified.  The applicant is responsible for assuring that a mailed application is received in Personnel Services by the filing deadline.  Do not send your application to or contact the hiring department or site.  Inquiries are to be made directly to the Personnel Services department.



Include all experience and education as required by the application.  It will be necessary for you to list separately the payroll title for each job at which you have been employed.  Also, specify the dates served on each job, the total years and months spent in each position, the salary received, and the reason for leaving the position.  Specific course work, where required by the Job Announcement Bulletin, should be clearly indicated.  If necessary, attach additional sheets.  Be sure to sign your application.

The minimum requirements for each position are noted in the job announcement.  Your application will be accepted only if it clearly indicates that the minimum requirements for the position are met.  It is your responsibility to assure that your application contains sufficient detail for us to determine your qualifications.  Your completed application is part of the examination process.  The information you give will be subject to review and verification.



Having a record of criminal convictions will not necessarily disqualify you from a position with the Mt. Diablo Unified School District.  However, false information or incomplete answers may result in rejection of your application or dismissal from employment.  Fingerprints will be taken and checked through the California Department of Justice.  No individual shall be allowed to begin employment until fingerprints are cleared.



The Authorization to Acquire Information from References form must be signed and submitted with your application giving us authorization to contact your previous or current employers regarding job performance.



Testing will be required for many positions to qualify the candidate for further consideration.  If testing is required, pertinent information will be noted under “Selection Procedures” on the job announcement.  Arrive promptly at the test site.  You will not receive a written or verbal reminder. 

All application materials will be reviewed and a maximum of ten applicants whose qualifications most clearly match the job requirements will be invited for an interview.



 After an official offer of employment has been made, you will be required to:

  • Take an Oath or Affirmation of Allegiance
  • Obtain a negative TB examination – employee bears the cost
  • Be fingerprinted – employee bears the cost
  • Provide proof of your right to legally work in this country
  • Most positions require a physical examination – employee bears the cost
