  • This form is required ONLY if your student wants to eat food prepared and served by the school kitchen.
  • These requests are high priority. Await meal accommodation approval before visiting your school kitchen for requested meals.

While all meals served by our program are peanut- and tree nut-free in terms of ingredients, many commercially produced items may be produced in a facility that processes nuts and other allergens.  As a result, we cannot guarantee that foods are completely free from trace amounts of nuts or nut residue.
