Transportation Requests

Transportation survey for elementary students for the 2024-2025 school year.

Click on the green link below and fill out the survey form if you have an elementary school student who lives more than one and a quarter (1.25) miles from their residential school and is interested in *school bus transportation. Please note that Sequoia Elementary and Monte Gardens Elementary are schools of choice** and not residential schools, and, therefore, their students do not qualify for this request.  If an intra-district transfer form was obtained to attend another school in the the district, the new school is not your residential school, and does not qualify.

*This is a survey to determine the need for transportation in a community. We will review the requests, and some requests may not be accommodated due to Transportation Department limitations. We will try to accommodate student transportation if qualifications are met. 

**School of Choice: This is referencing schools open to MDUSD students who reside in the MDUSD boundaries and have an approved intra-district transfer or to kindergartners who signed up for the "lottery for kindergarten" that received an acceptance letter. Those circumstances do not qualify for transportation.

Public bus pass survey for Mt. Diablo Unified School District middle and high school students for the 2024-2025 school year - *with mileage exceptions.

Click on the green link below and fill out the survey form if you have a middle or high school student who qualifies using the criteria below. This request form is not for students at Mt. Diablo High School and Olympic High School that receive bus transportation from the Bay Point area or those living within the previous Glenbrook Middle School area being transported to El Dorado Middle School. Please note that Sequoia Middle School is a school of choice (see above for description) and not a residential school; therefore, their students do not qualify for this request.  If an intra-district transfer or transfer form was obtained to attend another school in the district, the new school is not your residential school, and does not qualify. Charter school students do not qualify for this request. 

2024-2025 Public Bus Pass Survey for Mt. Diablo Unified School District Middle and High School Students 

  • Middle school students must live three (3) or more miles from their residential school.
  • High school students must live seven (7) miles or more from their residential school.

We will try to accommodate qualifying students quickly, but be advised that it could take up to two weeks for students to receive their public bus passes in the 2024-2025 school year. The passes will be sent to the student's school for distribution. Replenishment of bus passes is related to student attendance.

*For example, if a student was absent for two days during the last month, the student will receive four fewer bus passes (two bus passes each day) next month.
