Qualifying Life Events

Qualifying Life Events include:
- Marriage / Domestic Partnership
- Divorce / Legal Separation
- Birth / Adoption of a child
- Loss of other employer-sponsored coverage
- New election of other employer-sponsored coverage
*Qualifying Life Events duration is 60 days for all events.
Marriage / Domestic Partnership: You have 60 days from the date of your marriage or establishment of a domestic partnership to request a status change. To add your spouse or domestic partner, please contact our office at benefits@mdusd.org for the required enrollment forms. Submit the completed paperwork along with a copy of your marriage certificate or domestic partnership registration. The qualifying event date will be the date of marriage or domestic partnership.
Divorce / Legal Separation: You have 60 days from the date of your divorce or legal separation to request a status change. To remove your former spouse or legally separated spouse from coverage, please contact our office at benefits@mdusd.org for the required forms. Submit the completed paperwork along with a copy of your divorce decree or legal separation documentation. The qualifying event date will be the date of the divorce or legal separation.
Birth / Adoption of a Child: You have 60 days from the date of birth or adoption to request a status change. We encourage you to complete this process as soon as possible to avoid any delays in coverage. To add your child as a dependent, please contact our office at benefits@mdusd.org for the required enrollment forms. Submit the completed paperwork along with hospital birth confirmation (for a newborn) or adoption/court documents (for adoption). The qualifying event date will be the child’s date of birth or date of adoption. Please also provide a copy of the birth certificate and your child’s Social Security number once they become available.
Employee Lost Coverage or Dependent Lost Coverage: You have 60 days from the date your or your dependent’s coverage ends to make changes. To update your benefits, please contact our office at benefits@mdusd.org, and we will provide the necessary enrollment forms. Once we receive the completed forms along with the loss of coverage letter or memo, we will process your request and send you a health enrollment confirmation.
Employee or Dependent Gained Employment/Other Coverage: You have 60 days from the date your dependent’s new coverage takes effect to make changes to your benefits. To update your benefit coverage, please contact our office at benefits@mdusd.org. If the employee gains alternate medical coverage, they may be eligible for Cash-in-Lieu. Please email us and we will provide the required forms. Once we receive the completed forms along with the alternate insurance card as supporting documentation, we will process your request and send you a confirmation.
