PRESCHOOL ASSESSMENT CENTER

If you have a child between the ages of 3-4 years old who is not yet old enough for transitional kindergarten who you suspect may have a disability, please contact the District enrollment center to register your child for the purposes of assessment.

All new students enrolling to be assessed or to participate in our special education infant & preschool programs must first complete the online enrollment process. Once the online enrollment process is complete you will need to visit the Enrollment Center to verify your address and provide the necessary documents.

Proof of Residency

All registration documents are uploaded into your online enrollment application.

Once you provide the documents listed above, the Enrollment Center will provide you with an exit ticket including a QR code and a URL to complete a Parent Questionnaire. This questionnaire provides us with essential information to determine what kind of evaluation your child needs. We can not move forward with an assessment until all documentation is complete.

Parent Questionnaire

Incomplete enrollment packages will delay the assessment process.

For additional information about the preschool assessment and services, please call 925-685-1960 or email strongs@mdusd.org.
