Change of Address

Families that have moved are now invited to come into the Enrollment Center to update all their students' addresses at the same time. 

Moved but still living within MDUSD boundaries?

Please provide:

  1. Parent/guardian ID
  2. Two items from separate categories of the Board approved address verification list

School site administrators will review all intradistrict transfer, change of address requests for school and/or program space, in addition to attendance (including tardies), behavior, and academic progress. Continued attendance may be approved, conditionally approved, or denied based on the out come of the review. Denials must be reviewed by district administrators.

Immediate change in placement to the new school of residence is dependent on space available in the required grade level.

Intradistrict transfer are approved through the school level.

Students on any kind of transfer must maintain satisfactory attendance, behavior, and academic progress or their transfer may be revoked. 

Parents maintain responsibility for transportation.

Moved outside of the MDUSD boundaries?

Please provide:

  1. Parent/guardian ID
  2. Approved interdistrict transfer release from new district of residence

School site and district administrators will review all interdistrict transfer, change of address requests for school and/or program space, in addition to attendance (including tardies), behavior, and academic progress. Continued attendance may be approved, conditionally approved, or denied based on the out come of the review.

Approved interdistrict transfers MUST be renewed at each school level OR annually for students enrolled in specialized programs.

Students on any kind of transfer must maintain satisfactory attendance, behavior, and academic progress or their transfer may be revoked.
